What HCG 5000 IU is
HCG (human chorionic gonadotropin) is a hormone that acts like LH, the signal your brain sends to the testes to make testosterone and sperm. Each order is one vial of 5,000 IU freeze-dried powder, which you mix with bacteriostatic water before use. The water isn’t included; you’ll find it under Pairs well with below.
Doctors prescribe hCG for fertility treatment and for men whose testes don’t get enough signal from the brain. In the steroid world, it’s used to keep the testes working during a cycle or on TRT. Health Canada has warned that hCG is not authorized for weight loss, despite how widely it’s marketed for that.

How it works
When you use outside testosterone or steroids, your brain stops releasing LH and FSH. Without LH, the testes make little testosterone, shrink and produce less sperm.
- HCG stands in for LH. It binds the same receptor on the testes, so they keep producing testosterone and stay closer to normal size.
- It doesn’t replace FSH. HCG alone supports sperm production partly; full fertility treatment sometimes needs FSH too, under a specialist.
- It raises estradiol as well. Testes stimulated by hCG make more estrogen, which is why gynecomastia is a possible side effect.
HCG has a half-life of about 24–36 hours, so it’s usually injected two or three times a week.
Side effects and risks
HCG is generally well tolerated at low doses. Side effects become more common at high doses.
Gynecomastia
Higher testicular estrogen output can cause tender or swollen breast tissue. Lower, more frequent doses reduce this risk.
Water retention and mood
Bloating, headaches, irritability and tiredness are reported, mostly at higher doses.
Desensitization
Large doses can make the testes less responsive to LH for a while, which works against the reason you’re using it.
Injection site reactions
Mild redness or soreness under the skin. Use a new sterile needle every time and rotate sites.
For women
HCG is used in fertility treatment under close supervision. Without it, there’s a risk of ovarian hyperstimulation, a potentially serious condition.
Don’t use it if you
Have prostate cancer or another androgen-dependent tumour, a pituitary tumour, uncontrolled thyroid or adrenal disease, are under 18, or are pregnant.
Bloodwork checklist
Test before you start and 4–6 weeks in. Repeat after any change in dose.

Dosing information
Clinical reference. For men with hypogonadotropic hypogonadism, hCG labels give 1,000–2,000 IU two or three times a week. To help preserve fertility in men on testosterone, studies have used lower doses, such as 500 IU every other day.
Talk to a doctor before use; fertility clinics in Canada manage hCG regularly.
Mixing math. The water you add sets the strength. For example, adding 2 ml of bacteriostatic water to a 5,000 IU vial gives 2,500 IU per ml. On a U-100 insulin syringe:
- 250 IU = 0.1 ml = 10 units
- 500 IU = 0.2 ml = 20 units
- 1,000 IU = 0.4 ml = 40 units
Write the concentration on the vial so every dose is measured the same way.
Mixing and storage
- You’ll need bacteriostatic water (sold separately), alcohol swabs and U-100 insulin syringes.
- Wipe both vial tops with an alcohol swab.
- Inject the bacteriostatic water slowly down the inside wall of the vial.
- Don’t shake it; swirl gently until the powder dissolves and the liquid is clear.
- Keep the mixed vial in the fridge (2–8 °C) and never freeze it.
- Pharmaceutical hCG labels say to use the mixed vial within 30–60 days, depending on the product.
- Inject under the skin of the belly with a short insulin needle, rotating sites.

Common questions
Should I use hCG on TRT?
Many men on TRT use it to keep their testes active and preserve fertility. Whether you need it depends on your goals, especially whether you want children, so it’s worth discussing with a doctor.
Can hCG replace PCT?
Not on its own. HCG keeps the testes responsive, but the goal after a cycle is getting your brain to send LH again. That’s why SERMs such as Nolvadex or Clomid are usually part of recovery, and hCG is typically stopped before they start.
Does hCG help with weight loss?
No. Studies haven’t shown hCG helps with weight loss, and Health Canada has warned against using it for that.
How long does a 5,000 IU vial last?
At 500 IU three times a week (1,500 IU a week), a little over three weeks. At 250 IU twice a week, ten weeks, if it stays within the storage window after mixing.
