Isotretinoin tackles every main cause of acne at once:
- Shrinks oil glands and cuts oil production dramatically.
- Normalizes skin cell turnover, so pores clog less.
- Reduces acne bacteria and inflammation indirectly, by starving them of oil.
Acne often gets a little worse in the first weeks before it improves. A full course usually lasts 4–6 months.

Dosing information
Clinical reference. The usual dose is 0.5–1 mg per kg of body weight a day, taken with food, for about 15–20 weeks, aiming for a total course of about 120–150 mg per kg. Lower doses are sometimes used to reduce side effects.

Side effects and risks
Dry skin and lips are nearly universal. The serious risks need active precautions.
Severe birth defects
Even one dose during pregnancy can cause serious birth defects or miscarriage. Anyone who could become pregnant must use two forms of contraception from a month before until a month after treatment, with negative pregnancy tests.
Mood changes
Depression, anxiety and, rarely, suicidal thoughts have been reported. Stop and get help right away if your mood changes.
Cholesterol and liver
Triglycerides and cholesterol often rise, and liver enzymes may too. Steroid use adds to this.
Dryness
Very dry lips, skin, eyes and nose, with nosebleeds. Lip balm and moisturizer help; contact lenses may become uncomfortable.
Joints, vision and headaches
Back and joint aches, reduced night vision, and rarely severe headaches with vision changes, which need urgent care.
Don’t use it if you
Are pregnant, could become pregnant without reliable contraception, are breastfeeding, have liver disease, very high triglycerides, take vitamin A supplements or tetracycline antibiotics, or have a history of depression without medical supervision.

- Skip alcohol and vitamin A supplements.
