What Zionova Testosterone Enanthate is
Testosterone enanthate is testosterone attached to an enanthate ester, which slows its release from the injection site so a single shot lasts several days. It’s one of the most widely used forms of injectable testosterone, both in testosterone replacement therapy (TRT) and in bodybuilding. Each 10 ml Zionova vial holds 250 mg per ml, 2,500 mg in total.
Testosterone is the hormone your body already makes, but an outside dose shuts down your own production, raises your red blood cell count and partly converts to estrogen. This page covers what to expect, what to watch for and which blood tests to get.

03How it works
How it works
Testosterone binds to androgen receptors in muscle, bone and other tissue, increasing protein synthesis, nitrogen retention and red blood cell production. Three things shape how enanthate behaves:
- The ester sets the timing. Enanthate has a half-life of about 4–5 days, so levels peak a day or two after a shot and then taper. Splitting the weekly amount into two injections keeps levels steadier.
- Part of it becomes estrogen. The aromatase enzyme converts some testosterone to estradiol. You need some estrogen for your joints, mood and libido; too much causes water retention and breast tissue growth.
- Part of it becomes DHT. That’s the androgen behind acne, oily skin and scalp hair loss in men who are prone to it.

Half-life, in plain numbers
A half-life of about 4-5 days means half of what is in the body is gone after that time, then half of the rest, and so on. The ester decides how slowly the hormone leaves the injection depot, which is why long esters are injected less often than short ones.

Dosing information
Clinical reference. For men with low testosterone, the Endocrine Society’s guideline suggests 75–100 mg a week, or 150–200 mg every two weeks, adjusted using bloodwork and symptoms. Product labels allow a wider range of 50–400 mg every 2–4 weeks.
At 250 mg per ml, 100 mg is 0.4 ml, so use a syringe with clear 0.1 ml markings. Doses used for muscle building are usually well above replacement levels, and every risk on this page rises with dose.

Where it is injected
Testosterone Enanthate is an oil, so it goes deep into a large muscle with a regular needle: never just under the skin and never into a vein. These are the muscles standard nursing guidance uses.

- Lie on your side or stand. Put the heel of your hand on the bony knob at the top of the thigh (greater trochanter): left hand on the right hip, right hand on the left.
- Point your index finger at the bony front of the hip (anterior superior iliac spine) and fan your middle finger back along the hip crest to make a V.
- The site is the middle of that V.
Why it is used. Many nursing programs now teach it first. The muscle is thick, the fat over it is thin and fairly even, and no large nerve or vessel crosses the target area.
Careful. The landmarks take a few tries to find on your own body. A mirror or a helper makes the first ones easier.

- Sit down. Find the bony knob at the top of the thigh and the top of the kneecap.
- Divide the distance between them into three equal parts.
- Use the middle part, on the outer front of the thigh, roughly where a trouser seam would run.
Why it is used. The easiest large muscle to see and reach on yourself, with plenty of muscle to work with.
Careful. Stay on the outer side. The femoral artery, vein and nerve run along the inner front of the thigh. It can feel stiff for a day or two.

- Find the bony ridge at the top of the shoulder (acromion).
- Measure two to three finger-widths straight down from it.
- Inject into the thickest part of the muscle on the outer side of the arm, well above the armpit line.
Why it is used. Quick to reach and the usual place for vaccines. Good for small volumes.
Careful. A small muscle: larger amounts hurt and can leak back out. Going too low on the arm risks the axillary and radial nerves.

- Picture a cross dividing one buttock into four quarters.
- The traditional site is the upper outer quarter, close to the hip crest.

Why it is used. Still shown in many older guides because the muscle is so large.
Careful. The sciatic nerve and the superior gluteal artery are close by, and the fat layer is often thick enough that oil ends up in fat instead of muscle.

- Hip, thigh or shoulderLong-ester oils like this one tend to be smooth on the muscle, so all three common sites are used. Keep the shoulder for smaller volumes.

Side effects and risks
The risks below grow with dose and with how long you use it.
Shutdown and fertility
An outside dose tells your brain to stop signalling the testes (LH and FSH). Your own production drops, the testes can shrink and sperm counts fall, sometimes to zero. Recovery can take months. If you plan to have children, talk to a doctor first.
Thicker blood
Testosterone raises hematocrit, the share of your blood made up of red cells. High hematocrit thickens the blood and raises the risk of clots and stroke. It’s the most common reason doctors lower a TRT dose.
Heart and blood pressure
It can raise blood pressure, lower HDL (“good”) cholesterol and cause fluid retention. These effects climb quickly at higher doses.
Estrogen-related effects
Water retention, puffy or tender nipples and gynecomastia (breast tissue growth). Once tissue has grown it often doesn’t go away without surgery, so act early.
Injection safety
Use a new sterile needle and syringe every time, wipe the vial top and skin with alcohol, and never share. Redness, heat or swelling that gets worse after a couple of days can mean infection and needs a doctor.
Don’t use it if you
Are under 18, pregnant or breastfeeding, have or have had prostate or breast cancer, have untreated sleep apnea, a high red blood cell count, or serious heart, liver or kidney disease. In women, testosterone causes virilization, some of it permanent.

Androgens make warfarin work harder. INR can rise quickly and bleeding has been reported, so the blood thinner usually needs closer checks and often a lower dose.
- CautionInsulin and diabetes tablets
Anabolic steroids can lower blood sugar and change insulin sensitivity, so diabetes medicines may need to be adjusted.
- CautionPrednisone and other corticosteroids
Both hold on to salt and water. Together they make swelling and blood-pressure rises more likely, which matters most with heart, liver or kidney disease.
- Good to knowOral steroids at the same time
An oral 17-alpha-alkylated steroid adds liver strain and pushes HDL cholesterol down further on top of what this injectable already does.

Get tested before you start, 6–8 weeks in, and 4–6 weeks after you stop. On long-term TRT, test every 3–6 months. Book testosterone tests midway between injections so the result isn’t skewed by a peak.

- Complete blood count (CBC), especially hematocrit and hemoglobin
- Lipid panel: HDL, LDL, triglycerides
- Liver panel: ALT, AST

12After a course
Post-cycle therapy and support
Ongoing TRT is meant to continue, so it has no “after”. When a course does end, many people follow it with post-cycle therapy (PCT): a SERM such as tamoxifen (Nolvadex) or clomiphene (Clomid) to help restart natural production, usually begun once the enanthate has cleared, about two weeks after the last injection.
Because testosterone converts to estrogen, some people keep an aromatase inhibitor such as anastrozole (Arimidex) on hand. Use one only if bloodwork shows high estradiol along with symptoms; pushing estrogen too low causes joint pain, low libido and poor mood. You’ll find these options under Pairs well with below.

Common questions
What’s the difference between enanthate and cypionate?
Very little. Cypionate’s ester is slightly longer, so it lasts a little longer, but they’re dosed the same way and produce the same effects. Enanthate is more common worldwide; cypionate is more common in North American prescriptions.
How often should I inject testosterone enanthate?
With a half-life of about 4–5 days, once or twice a week. Twice a week (for example, Monday and Thursday) gives steadier levels and usually fewer estrogen side effects than one larger shot.
Where is it injected?
Into a large muscle, such as the glutes, the outer thigh or the shoulder. Many people on TRT inject smaller doses into the fat under the skin instead. Rotate sites each time.
How should I store it?
At room temperature, away from light; don’t refrigerate it. If you see crystals in cold weather, warm the vial in your hand or in warm water and swirl until it’s clear.
