01The basics
What Zionova Testosterone Cypionate is
Testosterone cypionate is the form of injectable testosterone most often prescribed in North America for testosterone replacement therapy (TRT); Depo-Testosterone is the best-known brand. The cypionate ester makes the hormone release slowly from an oil depot in the muscle, so injections are needed only once or twice a week. Each 10 ml Zionova vial contains 250 mg per ml, 2,500 mg in total.
It’s also the most common starting point for people using testosterone to build muscle. Either way, an outside dose replaces your own production, so knowing the side effects and getting regular bloodwork matter.

03How it works
How it works
Once released from the ester, cypionate is simply testosterone. It activates androgen receptors throughout the body, boosting muscle protein synthesis, red blood cell production and bone density.
- Long-acting: cypionate’s half-life is about 5–8 days. Levels peak within a couple of days of an injection, then drift down over the week.
- Converts to estradiol: some testosterone is turned into estrogen by aromatase. That’s normal and necessary, but at higher doses it can lead to bloating and breast tissue growth.
- Converts to DHT: this stronger androgen drives acne, oily skin and hair loss in men with a family history of baldness.

Dosing information
Clinical reference. Endocrine Society guidelines for low testosterone suggest 75–100 mg once a week or 150–200 mg every two weeks, then adjusting based on bloodwork. The prescribing label for testosterone cypionate lists 50–400 mg every 2–4 weeks.
At 250 mg per ml, 100 mg is 0.4 ml and 200 mg is 0.8 ml. Doses used for physique goals are usually several times higher than replacement, and every risk on this page rises with dose and length of use.

Where it is injected
Testosterone Cypionate is an oil, so it goes deep into a large muscle with a regular needle: never just under the skin and never into a vein. These are the muscles standard nursing guidance uses.

- Lie on your side or stand. Put the heel of your hand on the bony knob at the top of the thigh (greater trochanter): left hand on the right hip, right hand on the left.
- Point your index finger at the bony front of the hip (anterior superior iliac spine) and fan your middle finger back along the hip crest to make a V.
- The site is the middle of that V.
Why it is used. Many nursing programs now teach it first. The muscle is thick, the fat over it is thin and fairly even, and no large nerve or vessel crosses the target area.
Careful. The landmarks take a few tries to find on your own body. A mirror or a helper makes the first ones easier.

- Sit down. Find the bony knob at the top of the thigh and the top of the kneecap.
- Divide the distance between them into three equal parts.
- Use the middle part, on the outer front of the thigh, roughly where a trouser seam would run.
Why it is used. The easiest large muscle to see and reach on yourself, with plenty of muscle to work with.
Careful. Stay on the outer side. The femoral artery, vein and nerve run along the inner front of the thigh. It can feel stiff for a day or two.

- Find the bony ridge at the top of the shoulder (acromion).
- Measure two to three finger-widths straight down from it.
- Inject into the thickest part of the muscle on the outer side of the arm, well above the armpit line.
Why it is used. Quick to reach and the usual place for vaccines. Good for small volumes.
Careful. A small muscle: larger amounts hurt and can leak back out. Going too low on the arm risks the axillary and radial nerves.

- Picture a cross dividing one buttock into four quarters.
- The traditional site is the upper outer quarter, close to the hip crest.

Careful. The sciatic nerve and the superior gluteal artery are close by, and the fat layer is often thick enough that oil ends up in fat instead of muscle.

- Hip, thigh or shoulderLong-ester oils like this one tend to be smooth on the muscle, so all three common sites are used. Keep the shoulder for smaller volumes.
- Rotate sidesGoing back to the same spot before it has healed builds lumps and scar tissue. Most people rotate through at least four spots: both hips and both thighs.

Side effects and risks
Most problems with testosterone are dose-related, and most show up in bloodwork before you feel them.
Natural production stops
Your brain senses the extra testosterone and stops sending LH and FSH to the testes. Natural testosterone and sperm production fall, and the testes shrink. Fertility usually returns after stopping, but it can take months or longer.
High red blood cell count
Testosterone stimulates red blood cell production. If hematocrit climbs too high (polycythemia), blood gets thicker and the risk of clots rises. Donating blood or lowering the dose are the usual fixes.
Cardiovascular strain
Higher blood pressure, lower HDL cholesterol and fluid retention are common at doses above replacement levels. Watch your blood pressure and keep up regular cardio.
Gynecomastia and water retention
Excess estradiol can cause tender, swollen nipples and lasting breast tissue. Early bloodwork lets you adjust before it becomes permanent.
Sleep, skin and mood
Testosterone can worsen sleep apnea and cause acne, oily skin, hair thinning and irritability. Snoring or waking up exhausted is worth mentioning to a doctor.
Don’t use it if you
Are under 18, pregnant or breastfeeding, have prostate or breast cancer (or a history of it), untreated sleep apnea, high hematocrit, or significant heart, liver or kidney disease. Women risk virilization, including a permanently deeper voice.

- SeriousWarfarin and other blood thinners
Androgens make warfarin work harder. INR can rise quickly and bleeding has been reported, so the blood thinner usually needs closer checks and often a lower dose.
- CautionInsulin and diabetes tablets
Anabolic steroids can lower blood sugar and change insulin sensitivity, so diabetes medicines may need to be adjusted.
- CautionPrednisone and other corticosteroids
Both hold on to salt and water. Together they make swelling and blood-pressure rises more likely, which matters most with heart, liver or kidney disease.

An oral 17-alpha-alkylated steroid adds liver strain and pushes HDL cholesterol down further on top of what this injectable already does.

Post-cycle therapy and support
If you’re coming off testosterone rather than staying on TRT, post-cycle therapy (PCT) with a SERM such as tamoxifen (Nolvadex) or clomiphene (Clomid) is commonly used to help your own production restart. With cypionate’s long ester, PCT usually starts about two weeks after the final injection.
An aromatase inhibitor such as anastrozole (Arimidex) is sometimes kept on hand, but only bloodwork showing high estradiol with symptoms justifies using one. Driving estrogen too low causes joint pain, flat mood and low libido. See Pairs well with below.

Cypionate or enanthate: which should I choose?
They’re nearly interchangeable. Cypionate lasts slightly longer, but both are injected once or twice a week at the same doses. Pick the one you can get consistently.
How often do I inject testosterone cypionate?
Weekly is common on TRT; twice a week gives steadier levels and can reduce estrogen side effects. Every two weeks is possible, but levels swing more between shots.
How long until I notice it working?
Energy, libido and mood often improve within 3–6 weeks. Changes in muscle and body composition take longer, usually 3–6 months on replacement doses.
How should I store it?
