How it works
Oxymetholone increases protein synthesis and strongly stimulates red blood cell production, which is why it was developed to treat anemia.
- It’s 17-α-alkylated. That change lets it survive the liver when swallowed, and it’s also why it strains the liver.
- It acts like estrogen without becoming estrogen. Oxymetholone doesn’t aromatize, but it can activate estrogen receptors directly. That’s why water retention and gynecomastia are common, and why an aromatase inhibitor does little to prevent them.
- Short half-life: about 8–9 hours, so daily doses are usually split.
Side effects and risks
Anadrol’s risks are dose-dependent and build quickly. Keep courses short.
Liver damage
Liver enzymes often rise, and its prescribing label warns of peliosis hepatis (blood-filled cysts in the liver) and liver tumours with long-term use. Avoid alcohol, don’t combine with other oral steroids, and get a liver panel.
Water retention and blood pressure
Rapid weight gain is partly water. Along with it come higher blood pressure, puffiness and sometimes headaches. Monitor your blood pressure at home.
Cholesterol
HDL drops and LDL rises, sometimes sharply. Combined with high blood pressure, that adds up to real cardiovascular strain.
Red blood cells
It raises hematocrit strongly, the reason it treats anemia. In healthy people, thick blood raises the risk of clots and stroke.
Gyno, appetite and mood
Gynecomastia, nausea or loss of appetite, and irritability. Testosterone suppression is significant, and in women it causes virilization.
Don’t use it if you
Are under 18, pregnant or breastfeeding, have liver or kidney disease, heart disease, high blood pressure, or a history of prostate or breast cancer, or are already taking another oral steroid.
Bloodwork checklist
Test before you start, after 2–3 weeks, and 4–6 weeks after you finish. Anadrol’s effects on the liver and blood show up quickly, so test early.

Dosing information
Clinical reference. Where oxymetholone is prescribed for anemia, the label dose is 1–5 mg per kg of body weight a day, usually 1–2 mg/kg, given for at least 3–6 months under close medical monitoring, including regular liver tests.
One Zionova tablet is 50 mg. Doses used for physique goals are often 50–100 mg a day or more, and every risk on this page rises with dose and duration. Courses for those goals are usually kept short to limit liver strain.

Post-cycle therapy and support
Anadrol suppresses natural testosterone, and it’s rarely used alone; it’s usually part of a cycle with an injectable testosterone.
