How it works

HGH works directly on fat cells and indirectly through IGF-1, a growth factor your liver makes in response to it. Together they:

  • Release stored fat to be burned for energy
  • Support protein building in muscle, tendons and other connective tissue
  • Raise blood sugar by making cells less responsive to insulin

Injected HGH has a short half-life, about 3–4 hours, but IGF-1 stays raised much longer, so once-daily injections are standard. Changes in body composition build slowly, usually over months rather than weeks.

Side effects and risks

Most side effects come from doses that are too high, and many ease when the dose comes down.

Fluid retention

Puffy hands, feet and face are the most common side effect, especially in the first weeks or after a dose increase.

Joint pain and numbness

Aching joints and carpal tunnel symptoms: tingling, numbness or weakness in the hands. Both are signs the dose is too high.

Blood sugar

HGH causes insulin resistance. Over time, high doses can raise blood sugar into the prediabetes or type 2 diabetes range, so test fasting glucose and A1C.

Headaches and vision

Rarely, HGH raises pressure inside the skull. A severe or persistent headache, nausea or changes in vision mean stop and see a doctor.

Long-term high doses

Years of excess growth hormone can cause acromegaly-like changes: a growing jaw, hands and feet, and enlarged organs, including the heart. These don’t fully reverse.

Don’t use it if you

Have active cancer or a history of it (GH and IGF-1 can feed tumour growth), diabetic eye disease, are recovering from major surgery or a critical illness, are pregnant, or are under 18 without a specialist’s care.

Bloodwork checklist

Get tested before you start, 4–6 weeks in, then every 3–6 months. IGF-1 is the number doctors use to judge whether a dose is right.

Dosing information

Clinical reference. For adults with growth hormone deficiency, Endocrine Society guidelines suggest starting at 0.2–0.4 mg a day (about 0.6–1.2 IU) for people under 60, and 0.1–0.2 mg a day for older adults, then adjusting every 1–2 months based on IGF-1 and side effects. 1 mg of somatropin equals about 3 IU.

Mixing math. Adding 1 ml of bacteriostatic water to a 10 IU vial gives 10 IU per ml. On a U-100 insulin syringe:

  • 1 IU = 0.1 ml = 10 units
  • 2 IU = 0.2 ml = 20 units
  • 5 IU = 0.5 ml = 50 units

One 100 IU box lasts 100 days at 1 IU a day, or 50 days at 2 IU.

Mixing and storage

  • Wipe both vial tops with an alcohol swab.
  • Draw 1 ml of bacteriostatic water and inject it slowly down the inside wall of the HGH vial. The vacuum can pull the water in fast, so hold the plunger and let it in gradually.
  • Don’t shake the vial. Roll it gently between your hands until the solution is clear. If it stays cloudy or has particles, don’t use it.
  • Mix only one vial at a time.
  • Store unmixed vials in the fridge (2–8 °C), away from light.
  • Keep the mixed vial in the fridge too, and never freeze it.
  • Inject under the skin of your belly or thigh, and rotate sites.
