What Testosterone 400 by Genetix Pharma is
Genetix Pharma’s Testosterone 400 is an injectable oil that combines testosterone enanthate (200 mg/ml) and testosterone cypionate (200 mg/ml), 400 mg of testosterone per ml in total. Each 10 ml vial holds 4,000 mg.
At 400 mg/ml, it’s a concentrated option for people using higher doses who want less oil per injection. There’s no pharmaceutical version of this blend, so everything on this page is based on what’s known about testosterone itself.

03How it works
How it works
Both esters break off in the body to leave testosterone, which binds to androgen receptors and increases protein synthesis, nitrogen retention and red blood cell production.
- Timing: enanthate (half-life about 4–5 days) and cypionate (about 5–8 days) release at a similar pace, so the blend is injected once or twice a week, like either ester alone.
- Conversion: as with any testosterone, part of it becomes estradiol (driving water retention and gynecomastia risk) and part becomes DHT (acne, oily skin and hair loss in men prone to it).
- Concentration: more testosterone per ml generally needs more solvent, which can make injections sting more than standard 250 mg/ml oils.

Clinical reference. For men with low testosterone, medical guidelines suggest 75–100 mg a week or 150–200 mg every two weeks. Bulking doses are typically far higher, and every risk on this page goes up with them.

Where it is injected
Testosterone 400 is an oil, so it goes deep into a large muscle with a regular needle: never just under the skin and never into a vein. These are the muscles standard nursing guidance uses.

- Lie on your side or stand. Put the heel of your hand on the bony knob at the top of the thigh (greater trochanter): left hand on the right hip, right hand on the left.
- Point your index finger at the bony front of the hip (anterior superior iliac spine) and fan your middle finger back along the hip crest to make a V.
- The site is the middle of that V.
Why it is used. Many nursing programs now teach it first. The muscle is thick, the fat over it is thin and fairly even, and no large nerve or vessel crosses the target area.
Careful. The landmarks take a few tries to find on your own body. A mirror or a helper makes the first ones easier.

- Sit down. Find the bony knob at the top of the thigh and the top of the kneecap.
- Divide the distance between them into three equal parts.
- Use the middle part, on the outer front of the thigh, roughly where a trouser seam would run.
Why it is used. The easiest large muscle to see and reach on yourself, with plenty of muscle to work with.
Careful. Stay on the outer side. The femoral artery, vein and nerve run along the inner front of the thigh. It can feel stiff for a day or two.

- Find the bony ridge at the top of the shoulder (acromion).
- Measure two to three finger-widths straight down from it.
- Inject into the thickest part of the muscle on the outer side of the arm, well above the armpit line.
Why it is used. Quick to reach and the usual place for vaccines. Good for small volumes.
Careful. A small muscle: larger amounts hurt and can leak back out. Going too low on the arm risks the axillary and radial nerves.

- Picture a cross dividing one buttock into four quarters.
- The traditional site is the upper outer quarter, close to the hip crest.

Why it is used. Still shown in many older guides because the muscle is so large.
Careful. The sciatic nerve and the superior gluteal artery are close by, and the fat layer is often thick enough that oil ends up in fat instead of muscle.

Route for Testosterone 400Into the muscle (IM)
- Pick the big musclesConcentrated 400 mg/ml oils are among the ones people most often say leave the muscle sore. The hip and outer thigh handle them best; the shoulder is for small volumes only.
- Rotate widelyShort esters leave the body quickly, so they are injected more often. More injections means more spots in the rotation, so each one fully settles first.
- Warm it in your handRolling the vial between your palms for a minute thins the oil so it pushes more easily. Never heat it in water or a microwave.
- Wash your hands

Side effects and risks
Expect the risks of testosterone at higher doses. All of them rise with dose and duration.
Shutdown of natural testosterone
Outside testosterone switches off LH and FSH signalling. Your own production stops, testes shrink, and fertility drops until you recover, which may take months.
Thick blood
Hematocrit rises with dose. When it’s too high, the risk of clots and stroke goes up. A CBC partway through tells you where you are.
Heart health
Raised blood pressure, lower HDL and fluid retention all strain the heart. Track your blood pressure at home and keep doing cardio.
Gynecomastia
High estradiol can lead to tender nipples and breast tissue that may need surgery to remove. Bloodwork helps you catch it early.
Injection sites
Use new, sterile needles each time and rotate between large muscles. Worsening redness, heat or swelling a few days after an injection can mean an abscess and needs a doctor.
Don’t use it if you
Are under 18, pregnant or breastfeeding, have prostate or breast cancer (or a history of it), untreated sleep apnea, high hematocrit, or heart, liver or kidney disease. Women risk permanent virilization.

Androgens make warfarin work harder. INR can rise quickly and bleeding has been reported, so the blood thinner usually needs closer checks and often a lower dose.
- CautionInsulin and diabetes tablets
Anabolic steroids can lower blood sugar and change insulin sensitivity, so diabetes medicines may need to be adjusted.
- CautionPrednisone and other corticosteroids
Both hold on to salt and water. Together they make swelling and blood-pressure rises more likely, which matters most with heart, liver or kidney disease.
- Good to knowOral steroids at the same time
An oral 17-alpha-alkylated steroid adds liver strain and pushes HDL cholesterol down further on top of what this injectable already does.

After a course, most people wait about two weeks for the long esters to clear, then use post-cycle therapy (PCT) with a SERM such as tamoxifen (Nolvadex) or clomiphene (Clomid) to help restart natural testosterone. Why choose a 400 mg/ml testosterone?
Less oil per dose. At 600 mg a week, that’s 1.5 ml instead of 2.4 ml of a 250 mg/ml product. If you’re using replacement-level doses, a standard 250 mg/ml testosterone is easier to measure.
