What Zionova Thanos is

Thanos puts two erectile dysfunction (ED) medications in one tablet: 25 mg of sildenafil, the drug in Viagra, and 25 mg of tadalafil, the drug in Cialis. The idea is to pair sildenafil’s faster start with tadalafil’s longer window. Each bottle holds 50 tablets.

No sildenafil–tadalafil combination is approved in Canada, and the labels for both drugs advise against taking them with another ED medication, because their effects on blood pressure add up. Read the risks below before you decide whether it’s right for you.

How it works

Both ingredients block PDE5, the enzyme that breaks down cGMP, the messenger that relaxes blood vessels in the penis during arousal. They do the same job on a different timeline:

  • Sildenafil starts working in about 30–60 minutes and lasts about 4–6 hours. A heavy, high-fat meal slows it down.
  • Tadalafil takes about 2 hours to peak and can work for up to 36 hours, with a half-life of about 17.5 hours.
  • Together they don’t create two separate effects; they block the same enzyme harder for the first few hours, which is also when side effects peak.

Like any ED medication, Thanos doesn’t cause an erection without arousal, and it doesn’t raise your sex drive.

Side effects and risks

Two PDE5 inhibitors means two sets of side effects. Most are mild, but the blood pressure risks deserve attention.

Common side effects

Headache, flushing, a stuffy nose and indigestion from both drugs; back and muscle aches from tadalafil; and, from sildenafil, a blue tint to vision or sensitivity to light.

Never mix with nitrates

Any nitrate, including nitroglycerin for chest pain and “poppers” (amyl nitrite), combined with Thanos can cause a dangerous drop in blood pressure. Because of the tadalafil, wait at least 48 hours after a tablet before any nitrate.

Low blood pressure

Combining two PDE5 inhibitors lowers blood pressure more than either one alone. Alcohol, alpha-blockers (such as tamsulosin) and blood pressure medication add to it. Dizziness or fainting means stop.

Erection lasting over 4 hours

Priapism is rare but is a medical emergency that can cause permanent damage. Go to an emergency department right away.

Vision or hearing changes

Sudden loss of vision in one or both eyes, or sudden hearing loss or ringing in the ears, means stop and get medical help immediately.

Don’t use it if you

Take nitrates or riociguat, take any other ED medication, have heart disease or recently had a heart attack or stroke, have low or uncontrolled high blood pressure, severe liver or kidney disease, or are under 18.

Before you take it

Because Thanos combines two drugs, it’s worth more care up front than a single ED pill.

  • Try one drug first: many men do just as well on sildenafil or tadalafil alone, with fewer side effects
  • Heart and blood pressure: any heart condition, chest pain on exertion, or blood pressure outside the normal range
  • Medications: nitrates, alpha-blockers, blood pressure drugs, and drugs that raise PDE5 inhibitor levels, such as ritonavir, ketoconazole, erythromycin or clarithromycin
  • Grapefruit: raises the levels of both ingredients
  • A check-up: ED can be an early sign of heart disease, diabetes or low testosterone

Dosing information

Clinical reference. Taken on their own for ED, sildenafil usually starts at 50 mg about an hour before sex (range 25–100 mg), and tadalafil at 10 mg (range 5–20 mg). Each is taken no more than once a day.

A Thanos tablet has 25 mg of each: a low dose of sildenafil and a dose of tadalafil just above its usual 20 mg maximum. The combination hasn’t been studied or approved, so there’s no clinical dose for it. Never take more than one tablet in 24 hours, and don’t add any other ED medication.

Talk to a doctor before use, especially if you have a heart or blood pressure condition.

Getting the best results

  • Take it 30–60 minutes before sex.
  • Skip a heavy, high-fat meal beforehand; it slows the sildenafil.
  • One tablet per 24 hours, maximum.
  • Don’t combine it with Cialis, Viagra, Supermans or any other ED product.
  • Keep alcohol light.
  • Store at room temperature, away from moisture.
