What Zionova Sustanon 250 is

Sustanon 250 is a blend of four testosterone esters in one oil: testosterone propionate, phenylpropionate, isocaproate and decanoate. The short esters raise testosterone within a day or two, and the long ones keep it raised for weeks. Each 10 ml Zionova vial contains 250 mg of testosterone esters per ml, 2,500 mg in total.

The original Sustanon was developed by Organon for testosterone replacement and is still prescribed in many countries. It’s also one of the best-known testosterone products in bodybuilding. Whatever the reason you’re using it, the effects and risks are those of testosterone, so this page covers both, plus the bloodwork to get.

How it works

Every ester in the blend releases the same hormone, testosterone. What differs is speed:

  • Propionate is the fastest, active within about a day.
  • Phenylpropionate and isocaproate fill the middle, over several days to a week or so.
  • Decanoate is the slowest and keeps releasing for up to about three weeks.

The original pharmaceutical formula uses 30 mg propionate, 60 mg phenylpropionate, 60 mg isocaproate and 100 mg decanoate per ml. Once released, testosterone acts like your own: it builds muscle protein, raises red blood cell production, and partly converts to estradiol (estrogen) and DHT.

Side effects and risks

Sustanon carries the same risks as any testosterone. They grow with dose and with how long you use it.

Shutdown and fertility

Outside testosterone stops your brain from signalling the testes. Natural production and sperm counts fall, and the testes shrink. Because of the decanoate ester, Sustanon takes longer to clear than single-ester testosterone, so recovery starts later.

Red blood cells

Testosterone raises hematocrit. Too high, and the blood thickens, raising the risk of clots and stroke. A CBC partway through tells you where you stand.

Heart and blood pressure

Higher blood pressure, lower HDL cholesterol and fluid retention are common at doses above replacement. Check your blood pressure weekly.

Estrogen effects

Some testosterone becomes estradiol, which can cause bloating, tender nipples and gynecomastia (breast tissue growth) that may not reverse on its own.

Skin, hair and mood

Acne, oily skin, scalp hair loss in men prone to it, irritability and worse sleep apnea. The fast propionate ester can also make injections sting more.

Don’t use it if you

Are under 18, pregnant or breastfeeding, have or have had prostate or breast cancer, untreated sleep apnea, a high red blood cell count, or serious heart, liver or kidney disease. In women, testosterone causes virilization, some of it permanent.

Bloodwork checklist

Test before you start, at 6–8 weeks, and 4–6 weeks after your last injection. On long-term TRT, test every 3–6 months.

  • Hormones: total and free testosterone, estradiol (sensitive test), LH, FSH
  • CBC: hematocrit and hemoglobin
  • Lipid panel: HDL, LDL, triglycerides
  • Liver panel: ALT, AST
  • PSA if you’re over 40
  • Blood pressure: weekly at home

Dosing information

Clinical reference. Where Sustanon 250 is prescribed for low testosterone, the label’s usual dose is 1 ml (250 mg) every three weeks, adjusted to symptoms and bloodwork. Because the fast esters peak early and the slow one tails off, levels rise and fall a lot over those three weeks, so many people split the same total into smaller, more frequent injections for steadier levels.

Doses used for muscle building are usually well above replacement, and every risk on this page rises with dose.

Post-cycle therapy and support

If you’re ending a course rather than staying on TRT, post-cycle therapy (PCT) with a SERM such as tamoxifen (Nolvadex) or clomiphene (Clomid) is commonly used to help restart natural testosterone. Because the decanoate ester lingers, PCT usually starts about three weeks after the last Sustanon injection, later than with enanthate or cypionate.

An aromatase inhibitor such as anastrozole (Arimidex) is only worth using if bloodwork shows high estradiol with symptoms. You’ll find these options under Pairs well with below.

Common questions

Is Sustanon better than testosterone enanthate?

Not better, just different. Milligram for milligram, both deliver testosterone. Sustanon starts faster and lasts longer but gives less even levels; enanthate is simpler to dose steadily. Many people find a single ester easier to manage with bloodwork.

Why does Sustanon have four esters?

It was designed so one injection could cover about three weeks: the fast esters work early and the slow decanoate carries the tail. That made it convenient for TRT with infrequent clinic visits.

How often should I inject Sustanon?

The clinical label uses every three weeks. For steadier levels, many people inject smaller amounts every 3–4 days or twice a week.

Why does Sustanon sting?

The short propionate ester is known for causing more soreness after injection than longer esters. Injecting slowly into a large muscle and rotating sites helps.
