- Testosterone enanthate is the most widely used injectable steroid and a standard prescription for low testosterone.
- It’s a long ester: a half-life of about 4 to 5 days means one or two injections a week.
- Side effects are predictable and well studied, which is why it’s the usual first compound and the base most others are added to.
- Bloodwork is essential: hematocrit, estradiol, lipids and blood pressure are the numbers that matter.
If there’s one steroid to understand well, it’s testosterone enanthate. It’s been prescribed since the 1950s, it’s the testosterone most clinical research has used, and most steroid users start with it. This guide explains how it works, how it’s used medically in Canada, what to expect, the side effects, and how to monitor them.
What is testosterone enanthate?
It’s testosterone attached to an enanthate ester, dissolved in oil for intramuscular injection. The ester slows the release of testosterone from the injection site, so a single injection raises levels for several days. Once in the bloodstream, the ester is cleaved off and the hormone is identical to the testosterone your body makes.
In Canada, testosterone enanthate is a prescription drug used to treat hypogonadism (low testosterone). It’s also the compound most bodybuilders start with, because its effects and side effects are the best understood.
How it’s used medically in Canada
For low testosterone, Canadian doctors commonly prescribe testosterone enanthate or cypionate at around 100 to 200 mg every one to two weeks, or a smaller dose weekly, then adjust based on bloodwork. The aim is to keep levels in the normal range. Weekly or twice-weekly injections give steadier levels than larger doses every two weeks. If you think you have low testosterone, see our TRT in Canada guide.
What to expect
A classic study at UCLA (Bhasin, 1996) showed how strongly testosterone works on its own: men given 600 mg of testosterone enanthate weekly for 10 weeks gained muscle and strength even without training, and more with training. Higher doses produce larger effects, but also more side effects, and the relationship isn’t linear.
Typical effects people report:
- Strength gains within the first few weeks
- Muscle gain, some of it water early on
- Better recovery between sessions
- Increased libido and energy
Side effects
Estrogen-related
Testosterone converts to estradiol through the aromatase enzyme. Higher testosterone means higher estradiol, which can cause water retention, breast tenderness and, in some men, gynecomastia (breast tissue growth). Our guide to water retention explains how estrogen fits in.
Blood thickening
Testosterone stimulates red blood cell production. Hematocrit above about 54% increases the risk of clots and stroke, and it’s one of the most common reasons doctors adjust testosterone therapy.
Heart and blood pressure
At higher doses, HDL cholesterol falls and blood pressure can rise. See our guides to cholesterol and blood pressure.
Skin and hair
Acne and oily skin are common; hair loss can accelerate in men prone to it, because some testosterone converts to DHT.
Suppression and fertility
Your own testosterone production and sperm count drop. Testosterone enanthate was tested as a male contraceptive for this reason. Read the fertility guide before using it if children are in your plans.
Bloodwork in Canada
- Before: total testosterone, LH, FSH, estradiol, complete blood count, lipids, liver and kidney function, and PSA if you’re over 40
- During: repeat after six to eight weeks, with blood drawn midway between injections
- After: testosterone, LH and FSH to confirm recovery
