01The basics
What SM5 is
SM5 is a 5 mg vial of semaglutide, the same active ingredient as Ozempic and Wegovy. Semaglutide is a GLP-1 receptor agonist: it mimics a gut hormone that tells your brain you’re full and helps your pancreas release insulin when blood sugar rises. SM5 comes as a powder that you mix with bacteriostatic water before injecting it once a week.
Semaglutide works well, but it’s a serious medication. Most people get stomach side effects at first, it has rare but important risks for the pancreas, gallbladder and thyroid, and the dose has to start low and rise slowly. Because you mix and measure it yourself, accurate dosing is on you; the math is below.

How it works
Your gut naturally releases GLP-1 after you eat. Semaglutide copies that signal but lasts far longer, with a half-life of about 7 days, which is why it’s injected once a week.
- Slower stomach emptying: food stays in your stomach longer, so you feel full sooner and for longer.
- Quieter appetite: it acts on appetite centres in the brain, and many people notice fewer cravings and less “food noise”.
- Smarter insulin release: it boosts insulin only when blood sugar is high, so on its own it rarely causes low blood sugar.

05Clinical reference
Dosing information
Clinical reference. Semaglutide is always started low and raised every 4 weeks so your stomach can adjust. For weight management (Wegovy), the labeled schedule is 0.25 mg once a week for 4 weeks, then 0.5 mg, 1 mg and 1.7 mg, reaching a 2.4 mg maintenance dose. For type 2 diabetes (Ozempic), it’s 0.25 mg for 4 weeks, then 0.5 mg, rising to 1 mg or 2 mg only if needed.
If side effects are hard to handle, staying at a dose longer is normal. Talk to a doctor before use.
Mixing math. The water you add sets the strength. For example, adding 2 ml of bacteriostatic water to the 5 mg vial gives 2.5 mg per ml. On a U-100 insulin syringe:
- 0.25 mg = 0.1 ml = 10 units
- 0.5 mg = 0.2 ml = 20 units
- 1 mg = 0.4 ml = 40 units
One vial holds 20 doses at 0.25 mg, 10 at 0.5 mg or 5 at 1 mg. Write the concentration on the vial, and double-check ml versus units every time; mixing them up is a common cause of dosing errors.

Side effects and risks
Stomach side effects are common and usually ease with time. The rest are less common but need to be taken seriously.
Nausea and stomach upset
Nausea, vomiting, diarrhea and constipation, worst when starting or raising the dose. Eat smaller meals and stop at the first sign of fullness. Severe vomiting or diarrhea can dehydrate you and harm your kidneys.
Pancreatitis
Severe stomach pain that won’t go away, sometimes spreading to your back, with or without vomiting. Stop using it and go to an emergency department.
Gallbladder problems
Gallstones are more common, especially with fast weight loss. Pain in the upper right abdomen, fever, or yellowing of the skin or eyes needs a doctor.
Thyroid warning
Semaglutide caused thyroid C-cell tumours in rodents; whether it does in humans isn’t known. See a doctor about a lump in your neck, trouble swallowing or ongoing hoarseness.
Low blood sugar
Rare on its own, but common when combined with insulin or sulfonylureas such as gliclazide. Shakiness, sweating and confusion mean eat or drink sugar right away.
Don’t use it if you
Or a family member have had medullary thyroid cancer or MEN 2, you’ve had pancreatitis, you have type 1 diabetes, you’re pregnant, breastfeeding or planning a pregnancy within 2 months, or you’re under 18.

It contains the same active ingredient, semaglutide, but it’s a different product. Ozempic and Wegovy are prefilled pens from Novo Nordisk that click to a set dose; SM5 is a powder vial you mix and measure yourself.

In the STEP 1 trial, adults taking 2.4 mg a week alongside diet and exercise lost about 15% of their body weight on average over 68 weeks, compared with about 2% on placebo. Results vary widely from person to person.
